8/13/26 - Heywood Hospital Outpatient Registration is experiencing issues with receiving faxed lab and xray orders from our office.
If you want to be sure Heywood Outpatient Registration has your orders (1) Before you go to Heywood Outpatient Registration, call 978 630-5070 to pre-register and they will let you know if they have received the orders. (2) If they do not have the orders, you can - Print your orders from Mountain View Family Practice Athena Portal ( https://24634.portal.athenahealth.com ) In the portal: Go to MY HEALTH Then HEALTH REMINDERS Then you will see the lab orders If you click on each one, there is a blue PRINT hyperlink If you click on that, you can print each order individually and bring the paper orders to Heywood Outpatient Registration. or - Call the office and ask staff to print your orders which you can pick up and bring with you to Heywood Outpatient Registration Heywood Hospital is working to correct this issue. Mountain View Family Practice, PC was established March 1, 2010 by Dr. Gretchen Kelley and Dr. Kelly Hoisington to promote the health and well-being of the Templeton and Greater Gardner Massachusetts area by providing accessible, high-quality medical care for families. In April 2024, Dr. Hoisington retired after over twenty years of serving the community.
Mountain View Family Practice, PC employs Nurse Practitioners and a Physician Assistant who provide primary care and acute care to pediatric, adolescent and adult patients. They are able to diagnose and manage acute episodic and chronic illnesses, while emphasizing health promotion and disease prevention. Our Nurse Practitioners, Kelly Clow, FNP-BC, Alison Hietala, FNP-C and Physician Assistant, Sarah Gile, PA-C, work in collaboration with Dr. Kelley to assess, diagnose, treat and manage the patient’s health problems and needs.
